Food Service / Restaurant Complaints

If a complaint is filed against a food establishment then it must be inspected. The Food Inspection Team works closely with our nurses to investigate any reported or suspected food borne illness complaint.

How to File a Complaint
Please submit all complaints in writing by one of the following methods. Be sure to include a name and phone number so we can obtain any additional information we may need. The identities of those filing complaints with our office are protected, and are not considered open records.

1. Fill out the General Food Establishment Complaint Form or the Food Borne Illness Complaint Form. Mail the form to: WCCHD Food Services, 211 Commerce Blvd., Suite 111, Round Rock, Texas 78664.
2. Fax the completed form to (512) 248-7619.
3. E-mail Food Services. In your email include all the information requested in the complaint form.
